The primary purpose of the Certified Nurse Practitioner (CNP) for Hospice and Palliative Care is to provide comprehensive health assessments and demonstrate expert skill in the diagnosis and treatment of actual and potential health problems of the hospice and palliative care patient population. The NP will integrate education, research, management, leadership, and consultation into a clinical role and function in collegial relationships to influence the health environment within the scope of practice permitted by state and federal laws and regulations, the professional code of ethics, and professional practice standards. Knowledge of symptom and disease management, psychosocial issues, family dynamics, and team processes are required in this position. The NP is committed to the philosophy, goals and objectives of Aultman Hospice and Palliative Care.

RESPONSIBILITIES AND EXPECTATIONS

  • Collects and uses advanced comprehensive assessment, diagnostic, treatment planning, implementation, and evaluation skills to provide nursing care
  • Documents assessment, diagnosis, and management of treatment and care provided
  • Prescribes medications and treatments according to Ohio Board of Nursing APN scope of practice, state and federal regulations and Aultman policies
  • Collaborates with the Hospice and Palliative Care physicians to complete consultations, patient rounding and face to face visits
  • Collaborates with other health care professionals in the community to maintain continuity of care for the patient
  • Accepts accountability and responsibility for advanced practice and continued competence through formal and independent learning activities
  • Acts as an expert clinical resource for nursing staff and contributes to the annual education plan in collaboration with the Unit Director and Medical Director
  • Creates and maintains a safe, therapeutic environment through the use of risk management strategies and quality improvement activities
  • Acts as an advocate for patients in the health care system and helps to develop policies and procedures that promote the health and safety of the patient, family, and community
  • Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
  • Participates in the Quality Assessment Performance Improvement (QAPI) program
  • Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow
  • Brings creativity and innovation to nursing practice to improve care delivery
  • Promotes teamwork by demonstrating willingness to assist others in meeting the needs of all individuals and collaborating effectively with the interdisciplinary team and other departments throughout the organization
  • Complies with Aultman infection control and safety policies
  • Completes annual competency requirements
  • Consistently communicates requests, suggestions, concerns, or problems appropriately
  • Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by UD and/or hospice and palliative care physicians
  • Protects patient health information according to HIPAA Privacy Rule and related Aultman policies
  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation
  • Supports strategic organizational goals
  • Abides by the Aultman Code of Conduct at all times

QUALIFICATIONS

  • Graduate degree from an accredited educational institution with a major in a clinical area of nursing
  • Membership in professional nursing organization preferred
  • Current, valid nurse license from State of Ohio Board of Nursing
  • Current, valid Certification of Authority issued by the Ohio Board of Nursing
  • Current, valid Certificate to Prescribe issued by the Ohio Board of Nursing
  • Three (3) years of full-time equivalent experience in nursing, hospice nursing experience preferred
  • Excellent interpersonal and oral/written communication skills; strong organizational and problem-solving skills; ability to stimulate new ideas and create a positive work environment
  • Current BLS certification
  • Valid Ohio Driver's License
  • Must have own transportation and provide proof of current liability insurance
  • Basic computer skills

WORKING CONDITIONS

  • Hours of operation with shifts as assigned including occasional overtime, on-call or off-shifts (evenings and/or weekends) scheduled as necessary.
  • Subject to frequent interruptions and changes in priority of duties throughout the day.
  • May be required to travel in inclement weather
  • Subject to emergency and other crisis situations
  • Hazardous Exposure Category 1
  • Sitting/standing/moving about during working hours
