Job Description

Provides physical and psychiatric nursing assessments for clients, as well as community psychiatric support and mental health counseling. Provides education individually and in group settings, preventive care and disease mgmt. care. Makes appropriate referrals to Primary Care providers, and acts as a liaison between providers. Identifies areas of need and delivers appropriate medical treatment within the legal scope of practice.

Supervisory Responsibilities:

Essential Functions:

  • Assessment
  • Provide assessment to clients.
  • Complete initial health assessments and identify medical and psychiatric needs.
  • Refer clients to other agencies, as appropriate (Emergency Room, Medical Clinics, etc.)
  • Report medical needs to Case Manager and, if appropriate, Physician.

Treatment Planning & Delivery

  • Develops with client a mutually acceptable plan of action and an appropriate method for monitoring and evaluating progress.
  • Participate in phlebotomy and injection clinic.
  • Educate clients about medications (i.e., purpose, names, color, side effects, administration).
  • Assist clients with organization of medications.
  • Educate clients about healthy lifestyle issues, e.g., nutrition, general hygiene, dental hygiene, diabetes, etc.
  • Involve CPST’s and Physicians regarding clients’ need for help with medications.
  • Participate in weekly Treatment Team meetings (i.e., SAMI, CRU, MDO) to bring medical perspective.
  • Provide community psychiatric support and linkage to community services and supports.
  • Provide behavioral health counseling to identified clients.

Administration

  • Order and review new medications to Central Pharmacy, as well as return unused/changed medications.
  • Assist in linkage with medication from Patient Assistance Program.
  • Assist Case Managers and/or Physicians with Clinician-On-Call. Follow policies and procedures for handling crisis or dangerous situations.
  • Field phone calls about medications.
  • Collaborate with CSS/Physician to facilitate scheduling of appointments with clients.
  • Consult with pharmacists on medical insurance issues.

Functional Knowledge

  • Describe the philosophies, practices, policies, and outcomes of the most generally accepted and scientifically supported models of treatment, recovery, relapse prevention, and continuing care for mental health disorders, addiction, and other substance-related problems.
  • Describe the behavioral, psychological, physical health, and social effects of psychoactive substances on the user and significant others.
  • Recognize the social, political, economic, and cultural context within which mental health disorders and addiction and substance abuse exist, including risk and resiliency factors that characterize individuals and groups and their living environments.
  • Recognize the potential for substance use disorders to mimic a variety of medical and psychological disorders and the potential for medical and psychological disorders to coexist with addiction and substance abuse.
  • Excellent knowledge of the medical and pharmacological resources in the treatment of mental health problems and substance abuse disorders.
  • Excellent knowledge of and skills in conducting physical assessments of clients.
  • Excellent skills in managing the Central Pharmacy.
  • Understand diverse cultures and incorporate into treatment approaches the relevant needs of culturally diverse groups and people with disabilities.
  • Strong knowledge of current alliances and other Cuyahoga County social services providers: what they offer and who to contact.
  • Understand the variety of insurance and health maintenance options available and the importance of helping clients access those benefits.
  • Understand the need for and the use of methods for measuring treatment outcomes.
  • Understand and apply setting-specific policies and procedures for handling crisis or dangerous situations.
  • Computer skills: Word, Excel, Profiler

Education and Experience Requirements:

  • Registered Nurse Licensed Bachelors Degree

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer and standing, bending and moving.
  • Requires some exposure to communicable diseases or body fluids.
  • Ability to occasionally lift up to 20 pounds.

Work Environment:

Business hours generally Monday through Friday, however some evening & weekend hours may be necessary to meet all client services / programs. Position may also be required to respond to emergencies (on-call) depending on program / level of care.

Travel: Periodic travel within community and between locations

Job Type: Full-time
