Full Time, Day Shift, 8:00 am -4:30 PM M-F occasional weekend day

PURPOSE OF POSITION

A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Operations/Clinical Manager.

RESPONSIBILITIES & EXPECTATIONS

  • Understands and adheres to established agency policies and procedures
  • Provides physician prescribed physical therapy
  • Improves or minimizes residual physical disabilities of the patient
  • Returns the individual to optimum and productive living within the patient's capabilities
  • Participates with all other home care personnel in patient care planning.
  • Takes initial history and makes initial evaluation.
  • Documents accurate and complete notes of observations, treatments and care of patients in a timely manner as per agency policy
  • Documents the patient's response to the plan of care
  • Participates with all other health care personnel in patient care planning
  • Contributes to the evaluation of the patient's response to the plan of care through documentation and verbal communication with other members of the interdisciplinary team
  • Revises the plan of care as necessary
  • Directs and supervises Physical Therapy Assistant (PTA) and Home Health Aide (HHA) personnel as required
  • Performs all skilled procedures as ordered by the physician and according to the plan of care
  • Assists the physician in evaluating patient's level of function
  • Informs the physician(s) regarding patient's progress towards goals or changes in the patient's condition
  • Consults with the physician(s) regarding change in plan of treatment
  • Instructs patients and family/significant others in home programs and activities of daily living
  • Completes initial evaluations, including plan of care and goals, and submits to the physician for approval
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with agency defined time frames
  • Demonstrates respect for personal privacy and property
  • Supports the patient's and family's diversity
  • Attends all case conferences as scheduled
  • Participates in case conference meetings to discuss the need for involvement of other members of the interdisciplinary team
  • Performs other activities or tasks within the scope of practice of the physical therapist as requested by the Director of Clinical Operations
  • Meets daily average productivity standard established by the agency
  • Completes assigned responsibilities in time frame required by agency policies
  • Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
  • Participates in the Quality Assessment Performance Improvement (QAPI) program
  • Complies with agency infection control and safety policies
  • Completes annual competency requirements
  • Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel
  • Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow
  • Protects patient health information according to the HIPAA Privacy Rule and Aultman policies
  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation
  • Supports strategic organizational goals
  • Always abides by the Aultman Code of Conduct

QUALIFICATIONS

  • Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
  • Currently licensed in the state of Ohio
  • Two (2) years' experience, preferred
  • Ability to exercise initiative and independent judgment
  • Current BLS certification
  • Valid OH Driver's License
  • Must have own transportation and provide proof of insurance
  • Basic computer skills

WORKING CONDITIONS:

  • Hours of operation with shifts as assigned.
  • Subject to frequent interruptions and changes in priority of duties throughout the day.
  • Subject to emergency and other crisis situations
  • Hazardous Exposure Category 1
  • Sitting/standing/moving about during working hours (see attached Physical Requirements Addendum for details)
