Job Description

Kpili Service has a potential need for a Medical Records Technician to provide support to the 711 HPW & UAFSAM Aerospace Medicine Clinical Sciences Division Aeromedical Consultation Service Clinical Records Section at Wright-Patterson Air Force Base, OH.

DUTIES INCLUDE:

  • Review and verify component parts of medical records to ensure completeness and accuracy of diagnosis, operations, and special therapeutic procedures that must conform to AFI 41- 210, Clinical Records Operating Instructions, and Department of Defense Coding Guidelines.
  • Review record documentation, abstract, and apply unique ECG Library codes provided by the ECG Library, and enter this data into ECG Library provided software programs with 100% accuracy.
  • Review record documentation, abstract, and apply ICD-10-CM/CPT (all levels) codes and enter clinical visits into the Coversheet system.
  • Contact medical staff for clarification and additional information to ensure accurate coding assignment.
  • Provide documentation training to clinic providers/staff as directed by COR or appointed representative.
  • Responsible for initiating and carrying out a wide variety of Electrocardiogram (ECG) Library medical records/Information Management administrative and clerical duties associated with patient care and treatment in an outpatient observation unit/clinic.
  • Responsible for processing a minimum of 40 ECG Library encounters/records per day, or 10 encounters/records per hour, depending on the acuity of the visit for assembly/analyzing in accordance with current Air Force and Aeromedical Consultation Service (ACS) Instructions and maintained in accordance with (IAW) established section guidelines.
  • Monitor and maintain ACS ECG Library record accountability.
  • In addition, will conduct an extensive search and retrieve the medical records for accountability and processing files.
  • Retrieves ACS ECG Library records and ACS case review charts as required.
  • Maintains current coding and medical record theory reference material in IAW Air Force Instructions (AFI) 41-210.
  • Ensure that ACS ECG Library medical records be secured due to medico-legal potential IAW AFI 41-210.
  • Will comply with the Privacy Act, 38- USC 5701 and 38 USC 7332 IAW FAR Clauses 52.44-1 Privacy Act Notification and 52.2242 and Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Perform admission and disposition procedures for ACS ECG Library patient records IAW AFI 41-210 and ECG Library/Clinical Records Operating Instructions.
  • Perform periodic numerical and deficiencies reviews on record files to ensure medical records located in ACS ECG Library have reflected accurately in automated tracking system and/or out guide placed where the record should be filed IAW ECG Library/Clinical Records Operating Instruction.
  • Perform Release of Health Information (RHI) Functions.
  • Will not release any of the above information without prior written consent from patient IAW AFI 33- 332, Air Force Privacy Act Program and DoD 5400.7, Freedom of Information Act Program, and HIPAA.
  • Will take and pick up reports and records from designated boxes in ACS for provider completion at a minimum of 2 times a day, preferably by 0730 and 1430 hrs.
  • Retrieves incomplete records needed for immediate coding. Coded ECG Library records are to be taken/picked daily and twice a day (0730 and 1430 hrs).
  • Perform processing of ACS cardiac studies, electrocardiograms, treadmills, holter monitors, catheterization, and echocardiograms on rated and non-rated flying personnel and applicants for flight training received from worldwide medical facilities.
  • Verifies proper physician specialization required for review of tests received. Verifies and/or compares names, social security, age or date of birth, aero-ratings, aviation service code on all cardiac studies (through the MPC Query database) received from medical facilities for accuracy and completeness. Inputs patient's demographic information into the ECG Library Correspondence software program/database.
  • Ensures 100% accurate match for each document.
  • Maintain a straight numeric digit Case Number or charge-out filing system; checking for misfiled records.
  • Maintains charged out system, by monitoring, rotating, pulling, and counting charge outs, in and pulled from the files.
  • Conducts updated reviews of department out guides to determine accurate record location (tracer action).
  • Retrieves and accounts for medical records on ACS visits that will be evaluated on following Monday with at least a 99% accountability rate IAW Air Force policy.
  • Perform scanning and indexing of ACS records IAW ECG Library Operating Instructions.
  • Perform shifting of records and filing of incoming documents/miscellaneous paperwork/records as needed IAW ECG Library/Clinical Records Operating Instructions.
  • Demonstrate effective communication skills.
  • Present a professional and courteous approach using positive communication skills with physicians, staff, and coworkers.
  • Reproduce and forward copies of documents to other military treatment facilities daily IAW AFI 41-210.

REQUIRED SKILLS AND EXPERIENCE:

  • Two years of general office experience, in the operation of a personnel computer and familiarity with common office software including Microsoft Excel, Microsoft Word, and electronic mail systems, answering telephones, correcting grammar, spelling, syntax, and formatting to accurately prepare, review and edit written correspondence required to update files and records.
  • Possess knowledge of DoD/AF and civilian Medical Records Department regulations with regard to Compliance, Patient Safety, Performance Improvement, and Risk Management directives, including, in-service training, maintenance of records, shifting of records, the release of medical information, privacy act laws, record retirements, transcription, scanning and indexing, straight and terminal digit filing, read and apply medical records theories, to include attention to detail, understanding medical terminology, anatomy, physiology, and pharmacology.

  • Must be a US citizen.
  • Must be able to pass a Tier 1 background check (T1)/SF85I.
